UMass Memorial Health is now offering care to our MassHealth patients through a new Accountable Care Partnership Plan (ACPP) with Tufts Health Plan called “Tufts Health Together with UMass Memorial Health.” Enrollment launched on April 1, 2023.

An ACPP is a group of primary care providers (PCPs) who work with a managed care organization to create a full network that includes PCPs, specialists, behavioral health providers and hospitals who work together to provide members with high-quality and cost-effective health-care services, commonly referred to as an “ACO” (Accountable Care Organization). Our partnership with Tufts Health Plan will allow us to use this ACPP model of care to improve the health outcomes and experiences of our MassHealth patients and help us improve health disparities.
