New Patient Welcome Letter
As Family Medicine providers, we provide pediatric, adult and geriatric care. Additionally, we care for pregnant women, deliver babies, and provide several office-based procedures. We are excited for the opportunity to provide care for you and your family.
We offer routine preventative health services, management of chronic illnesses, and prenatal care. Additionally, we offer same day appointments for acute or urgent health needs you may have.
Our practice believes in the importance of taking care of the whole patient; so we also offer behavioral health services and can provide short-term therapy for our patients.
We are partnered with the University of Massachusetts Chan Medical School and are a training site for Family Medicine residents, medical students, and behavioral health providers. We are committed to training the future generation of health care providers. Our residents are physicians who see patients independently with supervision from a faculty physician. Residents serve as primary care providers to their own patients. At the end of their three-year residency, the care of their patients will be transitioned to a new resident familiar with your health needs. Our learners are valuable members of our team that provide fresh insight to the ever-changing healthcare environment.
To provide you with the best possible care we ask that you bring the following to your first appointment with us:
- Medical records from your previous primary care provider
- A list of names and contact information from any of your previous providers including specialists
- A list of all your prescriptions and over-the-counter medications or all your medication bottles
- Insurance Card (your primary care provider must be updated with insurance prior to new patient appointment)
- Government-issued ID
- Copay, if required
Regarding appointments at Fitchburg Family Practice Team:
- Please arrive at the check-in desk 15 minutes before your scheduled appointment time so you may have time to complete paperwork prior to your appointment.
- Patients who arrive more than fifteen minutes late may have to reschedule.
- If you are unable to make your scheduled appointment, at least 24 hours notification is required.
- You may cancel/reschedule your appointment by calling 978-343-5270.
- If you need an urgent appointment or have any prescription needs prior to your first appointment, please contact your current primary care provider.
