Guidelines for Completing Your UMass Memorial Health Registration
Unless it’s an emergency, please follow these guidelines.
To complete the UMass Memorial Health registration process, ensure insurance billing is accurate, and send results to the right doctors, patients or their guardians must provide correct information during registration.
Please bring the following items:
- Government-issued photo ID (Why we ask for this)
- Current insurance card
- Copayment (if required by patient's insurance company)
Referrals
Patients seeing a specialist whose health plan requires an insurance referral from their primary care physician (PCP) should contact their PCP at least one week before the appointment to ensure the referral is on file. Without the referral, the appointment may need to be rescheduled.
Copayments/Patient Balances/Deductibles
Please be prepared to pay copayments, patient balances, and deductibles at the time of your visit. Our staff can help you set up payment arrangements if needed. We accept cash, checks, MasterCard, and Visa, and offer payment plans for your convenience.
Self-Pay Patients
If you don’t have insurance, an enrollment counselor will contact you before your appointment to discuss screening for MassHealth and free care programs to help cover current and future hospital costs. For more information, call 508-334-9300.
