Subacute Rehab at Home
Skilled nursing and rehab in the comfort of your own home
Services
In addition to the care you need specific to your condition, the services offered through our Subacute Rehab at Home program are the same as you would find at a skilled nursing facility, and include:
- Medication delivery and in-home care from UMass Memorial Medical Center nurses using state-of-the-art digital devices
- Doctor visits via telemedicine or in person at scheduled intervals
- Remote monitoring of vital signs from our command unit
- On-call providers who can dispatch paramedics/EMS in emergencies
- Coordination and delivery of food, supplies, therapy, social work, and care coordination
- Respiratory care, including oxygen
- Diagnostic testing (labs, radiology)
- Assistance with transportation to outpatient testing or appointments
- Interpreter services at no cost to you in your preferred language
What You Can Expect
If you are admitted to the Subacute Rehab at Home program, here’s what you can expect:
- Your care team will meet you at home after you leave UMass Memorial Medical Center
- We will help you get settled and review your rehab care plan
- You will stay connected with your care team 24/7 through a tablet which will enable telehealth visits and quick connection to your care team.
- During your stay, we will work with you to coordinate any additional services you might need after being discharged from our program
Your Care Team
Your Subacute Rehab at Home care team provides personalized support, coordinating closely with your primary care provider and UMass Memorial Health specialists to ensure seamless, comprehensive care.
Your Medical Doctor or Advanced Practice Provider
This medical provider will oversee your care and check in with you regularly during your experience, either in-person or virtually.
Your Nurse
A registered nurse will visit you in your home or meet with you via telehealth at least once a day.
Your Paramedic
A specially trained paramedic may visit you during your experience, depending on your care plan.
Rehabilitation Therapists
Occupational Therapists, Physical Therapists and Speech Language Pathologists will be scheduled based on your treatment needs.
Cost
Grant funding will cover the program costs. You may need to pay co-pays and other costs for medications and transportation, depending on your insurance plan. These co-pays and costs will not be more than what you would pay if you went to a skilled nursing facility.
