Our Team Approach to Diabetes Care

Our patients receive care from a team of diabetes specialists including endocrinologists, nurse pracitioners (NP) and certified diabetes care and education specialists (CDCES). We collaborates with other specialty clinics such as:

• Diabetes eye care

• Podiatry

• Heart and vascular

• Kidney care

• Weight loss programs

• Diabetes education and nutrition

The Diabetes Center of Excellence offers personalized care for:

• Adult diabetes (type 1 and type 2): Our diabetes specialists work with your primary care physician to support the most important member of the care team – the individual with diabetes.
• Pediatric diabetes (type 1 and type 2): Services for your infant, child or adolescent are provided through our Children's Medical Center.
• Gestational diabetes: Specialized care for women who develop insulin resistance during pregnancy.

Care Coach Program

Are you struggling to keep your blood glucose in a healthy range? Our Care Coach program can help. Participants in our program successfully lowered their A1C and demonstrated a reduction in the distress caused by their diabetes by implementing behavioral, lifestyle, and medication changes, thanks to more frequent contact with a diabetes expert, called a care coach.

Working alongside your provider, your coach is your go-to resource for all your diabetes needs. You and your coach will discuss:

• Managing medications (e.g., medication changes, financial assistance, refills)
• Managing blood sugar (e.g., individual trends, approaches to keep in target range)
• Training on diabetes technology (e.g., continuous glucose monitoring)
• Encouraging better lifestyle habits (e.g., motivation, exercise, nutrition plans)

The Care Coach Program was developed in collaboration with Shields Health Care Group. To be eligible, you must complete diabetes education.
