Tier 1 Funding: Requests of $10,000 or Less

Now accepting applications for Tier 1 Funding opportunities at HealthAlliance-Clinton Hospital. Apply for grants up to $10,000 to support community health needs. Applications are available beginning on June 11, 2024. Tier 1 Application.

Criteria (revised as of 2022) is Based on:

Tier 1 Funding Details:

  • Grants of $10,000 or less.
  • Final round of Tier 1 Funding. 
  • Completion of online application including budget detail required.
  • Fund priorities based on new priority areas and target populations.
  • Cannot be for event sponsorship.
  • Grantees from the previous Tier 1 Round in April not eligible for additional Tier 1 funds. 
  • Does not preclude organization/agency from applying for Tier 2 funding.
  • Will consider how application is to support capacity building; and diversity, equity and inclusion initiatives if project advances efforts to address priority areas.
  • Capital improvement request will require landlord approval (if applicable).
  • Fiscal agent needs to be clearly identified and letter of support provided (if applicable).
  • Partnerships and sustainability plans are encouraged. 
  • Upon receipt of the grant award, funds need to be committed within 3 months of award and spent down within 12 months.
  • Additional financial information may be required before grant funds will be released.  A W-9 will be required.
  • A total of $50,000 is available for this round of funding.

Public announcement – Request for proposals opening date: Tuesday, June 11, 2024

Tier 1 online application deadline: Friday, July 12, 2024

Tier 1 awards announced: by Friday, August 2, 2024
 

Tier 1 Reporting Deadlines:

Grantees Funded on 4/8/2024

Six-month reporting deadline: 9/30/24

Annual report deadline:  4/28/25

Grantees Funded on 8/2/2024

Six-month reporting deadline: 1/31/25 

Annual report deadline: 8/1/25
