Criteria (revised as of 2022) is Based on:

• HealthAlliance-Clinton Hospital’s most current Community Health Needs Assessment
• 2022-2024 Community Benefits Strategic Implementation Plan
   

Tier 2 Funding Details:

• Grants up to $50,000.
• Fund priorities based on priority areas and target populations.
• Linkages to policy, systems, environmental change.
• All applications must have an equity approach.
• Partnerships and sustainability plans are encouraged. 
• Additional financial information may be required before grant funds will be released.  A W-9 will be required.
• Prior DON grantees are eligible.
   

Tier 2 Important Dates and Deadlines:

(Grant awards totaling approximately $300,000): 

Public announcement – Request for proposals opening date: Tuesday, June 11, 2024 

Tier 2 online application deadline: Friday, July 26, 2024

Tier 2 awards announced: by Friday, August 28, 2024 
 

Tier 2 Reporting:

Six-month reporting deadline: 1/31/25

Annual report deadline:  8/1/25
